FORMS

Every candidate (teen) needs to fill out two forms. It is up to the candidate- not the parent- to fill out the forms.


Sponsor Form

        Studies show that young people are more likely to stay connected to their faith with adult role models. That is one important reason why confirmation sponsors are an integral part of our preparation.

Role of a Sponsor 
Being a sponsor is an incredible honor and privilege, but what is his or her role? "Being a sponsor is a lifelong commitment. A sponsor takes on the role of a spiritual parent who 'brings the candidate to receive the sacrament, presents him to the minister for the anointing, and will later help him to fulfill his baptismal promises faithfully under the influence of the Holy Spirit' (Rite of Confirmation 5)." Catholic Answers

​Sponsor Requirements
  • a practicing Catholic who believes and lives by all that the Catholic Church professes to be true and is invested in his or her relationship with Jesus Christ
  • someone who has already completed the Sacraments of Initiation ​
  • at least 16 years of age
  • not a parent
    

Saint Form ​

         As Catholics, we choose confirmation saints for role models as well as to be someone who can pray for us from heaven.  When someone is declared by the Church to be a saint, the Church is proclaiming that this member of the faithful practiced heroic virtue during their life on earth, ‘the Church recognizes the power of the Spirit of holiness within her and sustains the hope of believers by proposing the saints to them as models and intercessors’ (CCC 828).  

​Choose a saint who can be your friend, guide, and mentor throughout your whole life. You don't have to choose a saint of the same gender. You don't have to choose a saint of the same gender.  So, who do you want?

​Proof of Baptism

Due Oct 4th 
  • If the candidate was baptized at St. Mary in Huntley, please fill out the month and year at registration. 
  • If the candidate was baptized elsewhere, please mail in or drop off a hard copy to the Youth Ministry office. These can also be collected at the September parent meeting, retreat, or the first session in October.
